Elles permettent d\u2019examiner les organes reproducteurs f\u00e9minins, comme le vagin, le col de l\u2019ut\u00e9rus, l\u2019ut\u00e9rus, les trompes de Fallope et les ovaires, afin d\u2019identifier des anomalies et de diagnostiquer des maladies<sup>1<\/sup>. Les examens r\u00e9alis\u00e9s avec une sonde endovaginale sont des examens internes qui impliquent d\u2019introduire une sonde dans le vagin \u00e0 une profondeur de 5 \u00e0 7,5 cm<sup>1<\/sup><\/p>\n<\/td>\n<td style=\"width: 30%;\">\n<p style=\"text-align: left;\"><span style=\"font-size: small;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-1253\" src=\"https:\/\/tristel.com\/ch-fr\/wp-content\/uploads\/sites\/17\/2021\/05\/Check-up-illustration.jpg\" alt=\"Examen d'\u00e9chographie endovaginale\" width=\"340\" height=\"212\" \/><\/span><\/p>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>&nbsp;<\/p>\n<h4 style=\"color: #ffab00; text-align: left;\"><span style=\"color: #373737;\">Pourquoi les sondes endovaginales n\u00e9cessitent une d\u00e9sinfection de haut niveau ?<\/span><\/h4>\n<h3 style=\"color: #ffab00; text-align: left;\"><span style=\"color: #009faf;\">1. Recommandations de contr\u00f4le des infections<\/span><\/h3>\n<p>Les recommandations de contr\u00f4le des infections suivent la classification de Spaulding. Elle repose sur le risque infectieux des dispositifs utilis\u00e9s dans les proc\u00e9dures sur des patients<sup>2<\/sup>. Lors de l\u2019insertion d\u2019une sonde endovaginale dans le vagin d\u2019une patiente, la sonde touche les muqueuses intactes du col de l\u2019ut\u00e9rus et de la paroi vaginale. Ainsi, selon la classification de Spaulding (Tableau 1.), les sondes endovaginales n\u00e9cessitent une d\u00e9sinfection de haut niveau apr\u00e8s utilisation<sup>2<\/sup>.<\/p>\n<p>&nbsp;<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-1386 size-full\" src=\"https:\/\/tristel.com\/be-fr\/wp-content\/uploads\/sites\/26\/2020\/12\/Tv-probes-fig-1.png\" alt=\"La classification de Spaulding : d\u00e9sinfection de haut niveau des sondes d'\u00e9chographie endovaginales\" width=\"2183\" height=\"1302\" \/><\/p>\n<p style=\"text-align: center;\"><span style=\"font-size: small;\"><strong>Table 1.<\/strong> La classification de Spaulding\u00b2<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3 style=\"color: #ffab00; text-align: left;\"><span style=\"color: #ff0a66;\">2. Risques de microperforation et de fuite du prot\u00e8ge sonde\u00a0<\/span><\/h3>\n<p>Les recommandations en vigueur dans le monde entier pr\u00e9conisent l\u2019utilisation d\u2019un prot\u00e8ge-sonde afin de r\u00e9duire les contaminations lors des proc\u00e9dures endovaginales<sup>3-12<\/sup>. La contamination reste cependant possible en raison des microperforations, d\u2019une rupture partielle ou compl\u00e8te du prot\u00e8ge-sonde pendant son utilisation ou d\u2019un mauvais placement du prot\u00e8ge-sonde sur la sonde (Figure 1.). Certains prot\u00e8ge-sondes du commerce utilis\u00e9s en \u00e9chographie poss\u00e8dent des taux de fuite inacceptablement \u00e9lev\u00e9s (jusqu\u2019\u00e0 81 %) et n\u2019offrent nullement une protection fiable contre les agents infectieux, en particulier les virus<sup>13<\/sup>.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter wp-image-1387\" src=\"https:\/\/tristel.com\/be-fr\/wp-content\/uploads\/sites\/26\/2020\/12\/Tv-probes-fig-2.png\" alt=\"Risques de microperforation et de rupture du prot\u00e8ge-sonde\" width=\"500\" height=\"385\" \/><\/p>\n<p style=\"text-align: center;\"><span style=\"font-size: small;\"><strong>Figure 1.<\/strong> Risques de microperforation et de rupture du prot\u00e8ge-sonde<\/span><\/p>\n<p>La contamination peut subsister m\u00eame lorsque la sonde endovaginale est prot\u00e9g\u00e9e et fait l\u2019objet d\u2019une d\u00e9sinfection de niveau interm\u00e9diaire ou de bas niveau<sup>14-17<\/sup>. Si une sonde endovaginale n\u2019est pas d\u00e9sinfect\u00e9e de fa\u00e7on appropri\u00e9e, si le prot\u00e8ge-sonde comporte des microperforations ou des ruptures ou s\u2019il n\u2019est pas plac\u00e9 de mani\u00e8re appropri\u00e9e, une transmission nosocomiale de patiente \u00e0 patiente ou de patiente au professionnel de la sant\u00e9 risque de survenir. L\u2019utilisation de d\u00e9sinfectants inappropri\u00e9s sur les sondes ne r\u00e9duit pas le niveau de contamination microbienne \u00e0 un niveau s\u00fbr pour une utilisation sur la patiente suivante. Des agents pathog\u00e8nes peuvent \u00e9galement \u00eatre pr\u00e9sents pendant une p\u00e9riode prolong\u00e9e en cas de d\u00e9sinfection insatisfaisante d\u2019une surface ou d\u2019un dispositif<sup>5<\/sup>. Par exemple, le papillomavirus humain (HPV) peut survivre jusqu\u2019\u00e0 sept jours sur les surfaces<sup>18<\/sup>.<\/p>\n<p>&nbsp;<\/p>\n<h3 style=\"color: #ffab00; text-align: left;\"><span style=\"color: #009faf;\">3. Contamination du fil et de la prise de la sonde endovaginale<\/span><\/h3>\n<table>\n<tbody>\n<tr>\n<td>\n<p style=\"text-align: left;\">Voici les \u00e9l\u00e9ments d\u2019une sonde endovaginale (Figure 2.) qui sont \u00e0 risque de contamination sanguine ou microbienne :<br \/>\n\u2022 Tige \u00e0 ins\u00e9rer, introduite dans le vagin de la patiente<sup>20,21<\/sup><br \/>\n\u2022 Manche du dispositif<sup>20,21<\/sup><br \/>\n\u2022 Fil et prise de la sonde<sup>20,21<\/sup><br \/>\nLa d\u00e9sinfection de haut niveau pr\u00e9vient la propagation des agents pathog\u00e8nes nocifs de patiente \u00e0 patiente, tout en prot\u00e9geant les professionnels de la sant\u00e9 pendant les proc\u00e9dures. Risques de spores bact\u00e9riennes dans l&#8217;environnement\u00a0<\/span><\/h3>\n<p>Les spores bact\u00e9riennes, comme <em>Bacillus subtilis<\/em>, sont pr\u00e9sentes dans notre environnement et sont consid\u00e9r\u00e9es comme un contaminant environnemental<sup>22<\/sup>. Il n\u2019est n\u00e9cessaire de conserver ou de d\u00e9contaminer les sondes endovaginales dans un endroit st\u00e9rile, ce qui pourrait recontaminer les dispositifs apr\u00e8s d\u00e9sinfection. Les spores bact\u00e9riennes sont consid\u00e9r\u00e9es comme les micro-organismes les plus r\u00e9sistants aux d\u00e9sinfectants (Figure 3) et la st\u00e9rilisation est n\u00e9cessaire pour d\u00e9truire des taux \u00e9lev\u00e9s de spores. Etant donn\u00e9 que les sondes endovaginales ne tol\u00e8rent pas les temp\u00e9ratures \u00e9lev\u00e9es utilis\u00e9es lors de la st\u00e9rilisation et qu\u2019elles peuvent \u00eatre endommag\u00e9es par la chaleur ainsi que par les produits chimiques agressifs, il est essentiel d\u2019avoir recours \u00e0 une solution appropri\u00e9e telle que la d\u00e9sinfection de haut niveau.<\/p>\n<p style=\"text-align: center;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-1390\" src=\"https:\/\/tristel.com\/be-fr\/wp-content\/uploads\/sites\/26\/2020\/12\/Tv-probes-fig-4.png\" alt=\"R\u00e9sistance des micro-organismes aux d\u00e9sinfectants\" width=\"900\" height=\"472\" \/><\/p>\n<p style=\"text-align: center;\"><span style=\"font-size: small;\"><strong>Figure 3.<\/strong> R\u00e9sistance des micro-organismes aux d\u00e9sinfectants. Adapt\u00e9 de Centers for Disease Control and Prevention (2008)<sup>2<\/sup><\/span><\/p>\n<p>&nbsp;<\/p>\n<h4 style=\"color: #ffab00; text-align: left;\"><span style=\"color: #373737;\">La d\u00e9sinfection de haut niveau des sondes endovaginales r\u00e9duit le risque de transmission de patiente \u00e0 patiente et de patiente \u00e0 professionnel de la sant\u00e9<\/span><\/h4>\n<p>&nbsp;<\/p>\n<p>Une \u00e9tude de Meyers et al. (2014) a d\u00e9montr\u00e9 que les d\u00e9sinfectants de haut niveau mentionn\u00e9s dans les recommandations mondiales de d\u00e9contamination ne sont pas efficaces pour d\u00e9truire le HPV<sup>23<\/sup>. Jusqu\u2019\u00e0 peu, il n\u2019\u00e9tait pas possible de tester l\u2019efficacit\u00e9 des d\u00e9sinfectants contre le HPV natif. En l\u2019absence de m\u00e9thodes disponibles, les organismes de r\u00e9glementation recommandent d\u2019effectuer des tests sur un polyomavirus SV40 de substitution, utilis\u00e9 comme indicateur d\u2019efficacit\u00e9 contre le HPV. Cependant, le profil de r\u00e9sistance aux d\u00e9sinfectants compar\u00e9 des deux virus n\u2019a pas \u00e9t\u00e9 \u00e9tudi\u00e9. Cela signifie que l\u2019efficacit\u00e9 contre le polyomavirus SV40 de substitution n\u2019\u00e9quivaut pas forc\u00e9ment \u00e0 l\u2019efficacit\u00e9 contre le HPV23.<\/p>\n<p>La Soci\u00e9t\u00e9 de M\u00e9decine Maternelle et F\u0153tale (Society for Maternal-Fetal Medicine) a r\u00e9cemment publi\u00e9 (2020) des recommandations concernant la s\u00e9curit\u00e9 des patientes. Ces recommandations portent sur la r\u00e9duction du risque de transmission d\u2019infections lors des examens r\u00e9alis\u00e9s avec une sonde endovaginale et pr\u00e9conisent de respecter les pratiques suivantes:<\/p>\n<p>\u2022 Un usage unique des prot\u00e8ge-sondes pour chaque examen avec sonde endovaginale<sup>1<\/sup><sup>3<\/sup>.<br \/>\n\u2022 Une utilisation de poches de gel d\u2019\u00e9chographie \u00e0 usage unique st\u00e9riles<sup>1<\/sup><sup>3<\/sup>.<br \/>\n\u2022 Le nettoyage, c\u2019est-\u00e0-dire l\u2019\u00e9limination des \u00e9l\u00e9ments contaminant comme le gel et les d\u00e9bris qui peuvent r\u00e9duire l\u2019efficacit\u00e9<br \/>\nde la d\u00e9sinfection sur les sondes endovaginales apr\u00e8s chaque examen<sup>1<\/sup><sup>3<\/sup>.<br \/>\n<strong>\u2022 La d\u00e9sinfection de haut niveau \u00e0 l\u2019aide d\u2019un agent dont l\u2019efficacit\u00e9 contre le HPV a \u00e9t\u00e9 prouv\u00e9e<sup>1<\/sup><sup>3<\/sup>. <\/strong><\/p>\n<p>&nbsp;<\/p>\n<h5 style=\"text-align: center;\"><strong><a href=\"\/fr-fr\/blog\/un-test-novateur-prouve-que-le-dioxyde-de-chlore-de-tristel-est-efficace-contre-le-hpv-en-30-secondes\/\" target=\"_blank\" rel=\"noopener\">Une \u00e9tude scientifique de Meyers et al.
